Employees are eligible to participate in a group health and life plan on the first of the month following 90 days of full-time employment. The employee is responsible for meeting the enrollment requirements.

MAJOR HEALTH INSURANCE

In-network benefits: $5,000 deductible per person (up to $10,000 per family)

Co-Insurance: 80/20

Primary and Specialty Care Office Copay: $30

Prescription Drug Benefit:

Cost:

Individual Plan: $33.00 weekly
Employee/children: $71.00 weekly
Employee/spouse: $103.00 weekly
Family Plan: $141.00 weekly

DENTAL INSURANCE

Individual Plan: $6.54 weekly
Family Plan: $17.78 weekly

OTHER BENEFITS

401k: Company will match 50% of the employee’s contribution up to the first 6% of the employee’s annual income. Drivers are eligible after one year of employment.

Paid Vacation: After one year One week paid vacation

Weekly Settlements: All trip envelopes received by Wednesday noon will be paid on the following Monday and Direct Deposited into your account, trip packs provided

Driver Referral Bonus: $1250 bonus per driver referral paid in three months increments

Rider Program: $25.00 monthly (passengers must be 18 years of age to qualify for rider program)

Pet Policy: $350 deposit
